Each insurance carrier that is newly approved by the Florida Office of Insurance Regulation to write workers’ compensation insurance, Line of Business 160, must register with the Division of Workers’ Compensation before writing such insurance.
In accordance with section 440.52, F.S., each insurance carrier who desires to write Workers' Compensation insurance in compliance with this chapter shall be required, before writing such insurance, to register with the Division. Once the registration forms are electronically sent to the Division, a specific carrier identification number will be assigned to your company.
Once a carrier gets a Certificate of Authority the Division will issue a welcome letter with a username and password. The carrier will need to register online at https://apps.fldfs.com/Survey/default.aspx?qid=87. At that time the Division will issue a Carrier ID.
The Florida Office of Insurance Regulation will give the carrier a Certificate of Authority.

The Division of Worker's Compensation uses this registration information when assigning insurer identification numbers, tracking medical and indemnity claims, and to effectively communicate and disseminate penalty and Division information to newly registered insurers in a timely fashion.
