An investigator with the bureau may send you a contact letter if:
- You filed a complaint with the Bureau against an agent, agency, or other firm or individual regulated by the Division of Insurance Agent and Agency Services, or
- You filed a complaint with another government office that has been referred to the Bureau for investigation, or
- You were identified as a possible victim as a result of a suspected statutory or administrative violation or violations committed by a licensee.

In many instances, the investigator will request you give a sworn statement providing details of the alleged violation or violations committed against you. You will also be asked to provide any physical evidence (documents, receipts, etc.) that supports your complaint; copies may be made to be used as evidence in the investigation.
Your cooperation is an essential element of the investigation. The investigator will work with you to arrange an agreeable time and place to take your statement. You will be asked if you are willing to testify in the event the investigation leads to an administrative hearing to adjudicate the matter.
All investigators with the Bureau are assigned credentials to establish their identity and authority. The investigator or investigators will present their credentials (sometimes called a "badge") upon arrival at the site where your statement will be taken.
