Chapter 119, F.S., Florida’s Public Records Law and Civil Rules of Procedure require the release of certain information for public inspection upon request. There is a dedicated email address used for submitting public records requests; in addition, requests can also be submitted via mail, fax, telephone, and personal walk-ins. Once the Division has received a public records request, the Division will acknowledge the request within 24 hours of receipt to advise the Division is reviewing it to determine if we hold the records requested. Once records are identified responsive to the request, the records will be printed, invoiced if applicable, inspected for confidentiality, and redacted. Pre-payment is required prior to release of the records.

All requests for Division Assigned Numbers (DAN) must be in writing (email, fax, or letter) and the following information must be provided: injured worker name; date of accident; reason for request, i.e., pursuant to Administrative Rule 69L-3.003, FAC, the Division Assigned Number is needed to file the First Report of Injury or Illness (or specify other division form); and, name of individual and company name requesting number. If available, please provide the injured worker's date of birth, address and/or employer name.
